- Download the Child Bond Application Form . This interactive application form can be completed by typing directly into the boxes, so there's no need to fill it all out by hand
- Print the application form, then sign and date it correctly
- Post the completed application form to Scottish Friendly Assurance, Freepost, Glasgow, G2 4BR
