Let us know
When the person holding a policy with Scottish Friendly has died, you can notify us and make a claim by completing a death claim form.
What you need to send us
In addition to the above death claim form, we will require a valid copy of the death certificate and the original policy documents if available.
We will also let you know if you need to send us a nomination form, grant of confirmation/probate and a deed of assignment.
